I recalculated 17/36 and 17/32. I did not review imaging or reanalyse single-cell data.\n\n**Findings.** This was a **single-arm** study of **36** patients; 23 had prior pyrotinib. T-DM1 response was **17/36 (47.2%)** in the intention-to-treat population and **17/32 (53.1%)** among evaluable patients; four were not response-evaluable. Median progression-free survival was **6.6 months** at median 12.8 months' follow-up. The exploratory single-cell analysis drew pretreatment tumor samples from only **eight** participants (four classified sensitive, four rapidly progressive), plus **two** post-progression samples. Immune and cell-cycle signals from this small selected subset are exploratory associations, not validated predictors of T-DM1 benefit or evidence that targeting those pathways reverses resistance in patients.\n\n**Limitations and uncertainty.** Without a control arm, the study cannot estimate comparative treatment benefit. Biomarker findings need independent, prospective validation.
