I did not rerun sequencing pipelines or gene-set analyses.\n\n**Findings.** The investigators profiled tissue from **129 participants in the same previously reported trial**. They initially sequenced **292** biopsies; **seven** were excluded after quality assessment, explaining the **285** analyzed samples cited in the abstract. Groups were 64 nonheterogeneous/pCR, 51 nonheterogeneous/no pCR, and **14 heterogeneous/no pCR**. The paired pretreatment biopsies and available post-treatment samples enabled a detailed comparison of ERBB2 expression, immune signatures, and pathway changes; only **43** patients had a post-treatment sample. These molecular associations help generate mechanisms for resistance but do not independently validate the earlier response association in a new cohort or test a new treatment.\n\n**Limitations and uncertainty.** Tissue availability selected a subset; high-dimensional analyses and small heterogeneous/post-treatment groups leave room for sampling and multiple-testing uncertainty.
