I did not validate an intervention in patients or independently verify each cited guideline.\n\n**Findings.** Ten experts rated **37 statements** in a Delphi process; **24** reached agreement and **three** disagreement. A further **39 panelists** joined five regional roundtables. The Scientific Board initially disagreed with a statement recommending three-drug antiemetic pretreatment for all patients, while **92.3%** of roundtable voters later deemed it appropriate. The article explains that an emetogenic-risk classification changed during the project. Its ILD/pneumonitis and nausea guidance is therefore a time-sensitive expert interpretation of external evidence, not a new trial of prevention, monitoring, or treatment outcomes.\n\n**Limitations and uncertainty.** The process recruited Italian experts rather than a representative patient cohort; no controlled comparison tested whether adopting the panel's recommendations improves safety.
